Assessed disclosure
The Department makes a wide variety of information publicly available. You may be able to find what you are looking for on our Routine Disclosure Page, or on one of our other websites, listed below in the footer of this web page.
The Department also has a special process in place to request information about your licence, vehicle registration, or a motor vehicle accident you were involved in. Visit Transport Tasmania's information request page.
If you can’t find the information you want, you can make an application for assessed disclosure. Applications must be in writing. An on-line form is available here. If you would prefer, a printable version is available:

You do not need to use these forms, however your application must be in writing and must include the information that is requested on the form. This is a requirement under the RTI Regulations.
Applications must be accompanied by the application fee, which is currently $47.75 for 2025-2026. You can pay using the following methods:
- Cheque or Money Order to Department of State Growth
- Direct deposit to WESTPAC BSB 037-001, Account 259881
- Credit card – by calling Finance Branch 1800 030 688
You may request that the fee is waived on the following grounds:
- you are in financial hardship - we will need evidence such as a Centrelink Health Care Card
- you are a Member of Parliament, and the application is in connection with your official duty
- you are a journalist, and the application is in connection with your professional duties
- you can show that you intend to use the information you seek for a purpose that is of general public interest or benefit.
